Job Description: 

Fort Drum Regional Health Planning Organization (FDRHPO) is seeking a dynamic, self-motivated individual to lead community-based initiatives designed to improve health outcomes, address health disparities, and strengthen partnerships across healthcare and community partners. The Community Health Project Manager will facilitate regional planning, implementing, coordinating, and evaluating for projects across Jefferson, Lewis, and St. Lawrence counties.

Responsibilities: 

• Lead the implementation and evaluation of population health initiatives and improvement projects. 
• Adhere to workplans, timelines, budgets, and performance measures to ensure successful project outcomes.
• Analyze data, monitor program activities, and recommend improvements based on outcomes and community needs for quality improvement and program enhancement. 
• Build and maintain collaborative relationships with healthcare providers and community-based organizations.
• Facilitate community partner meetings, workgroups, focus groups, and promote stakeholder engagement in population health initiatives.
• Develop, monitor, and manage subcontracts for population health initiatives. 
• Provide guidance and technical assistance to subcontractors and key stakeholders.
• Coordinate outreach activities designed to increase awareness of population health initiatives. 
• Manage grant-funded projects and ensure compliance with funding requirements.
• Track program objectives, deliverables, and outcomes, maintain accurate documentation, and complete grant reporting requirements. 
• Proactively identifies organizational needs, determines appropriate actions, and executes solutions independently.

Qualifications: 

• Experienced professional with initiative, who is goal-oriented and outcomes-driven. 
• Bachelor’s degree in Public Health, Health Administration, Human Services, or a related field. 
• Proven ability to independently manage a large project or grant with exceptional organizational detail and adherence to deadlines. 
• Exhibits excellent communication, teamwork and interpersonal skills with a professional demeanor.
