Psychiatric Rehabilitation Coordinator Job Description
Summary: The Psychiatric Rehabilitation (PRP) Coordinator promotes and supports the mission of the PRP in assisting individuals with serious mental health conditions improve their functioning. The PRP Counselor functions as a direct care worker and is directly involved with the clients’ progress in achieving identified goals and works under the Supervision of the Rehabilitation Specialist.
Essential Responsibilities:
- Perform a needs assessment and coordinate in the development and implementation of the Individual Rehabilitation Plan (IRP).
- Develop and maintain contact notes and monthly or quarterly summaries.
- Prepare discharge recommendations and summaries.
- Assist client in meeting self-care, social skills, independent living, and health promotion goals.
- Assist with applying for benefits and resources in the community
- Coordinate appointments
- Participate in clinical training, supervision treatment teams, and other clinical and educational activities.
- Travel and transport client in the community in personal vehicle (must have a valid driver’s license and auto insurance).
- Client visits (Six per month for adults; 3 per month for youth)
- Perform life skills training and groups.
- Provide crisis intervention.
- Ability to prepare formal reports and record pertinent information regarding the client’s status.
Qualifications:
- Excellent interpersonal skills.
- Computer skills with the ability to use technology in the delivery of care.
- High school diploma or equivalency
- 40 hours of PRP training and/or coursework in social sciences
- Ability to work as a team member
- Criminal background check completion required
- Must be free from chemical dependency and maintain this for the duration of providing services or employment. If there is a previous history of chemical dependency, you must have one year of freedom from chemical dependence preceding employment.

Compensation
$20 per hour
