Population Health Analyst

JOB DESCRIPTION/PERFORMANCE EVALUATION

 

DEPARTMENT:                  Population Health, Ambulatory Care             

POSITION TITLE:              Population Health Analyst I

REPORTS TO:                     Director                       

 

PRINCIPAL MISSION:

Under the direction of the Director of Ambulatory Care Quality and Growth, the Population Health Analyst I will design and implement clinical and financial analyses to improve patient care and meet strategic business goals. The Data Analyst will use a data-driven approach to support Quality and Performance Improvement program activities and guide population health and value-based initiatives in Ambulatory Care.

 

This position collaborates with other stakeholders to gather data, run analytic reports, and contribute to the analysis and interpretation of results. This includes reviewing patient charts to identify gaps in care and developing reports and analytics to support frontline staff and clinical operations. Candidates must have knowledge of healthcare settings and be able to communicate analysis findings to internal stakeholders.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

RESPONSIBILITIES

1. Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).

2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct. 

3. Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion. 

4. Demonstrates the standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.

5. Collaborate with leadership, practice management, IT, and other stakeholders to collect and analyze data to improve the quality of clinical care.

6. Support the collection and transfer of data with internal and external IT stakeholders.

7. Manage daily, weekly, and monthly data-management tasks.

8. Run dashboards, reports, and data analysis using data extracts from a range of sources.

9. Produce and distribute pre-visit planning reports to clinical and patient-facing teams to guide patient care and improve care quality and performance.

10. Review electronic medical records to confirm if patients are missing services, and develop reports for scheduling, or a supplemental data report where care has already been provided.

11. Communicate the findings of reports, explaining technical results to front-line staff.

12. Conduct analyses as requested by the Director.

13. Support special quality improvement projects in Population Health and Ambulatory Care services and take ownership for agreed project activities.

14. Play an important role on the Population Health cross-functional team, helping to develop recommendations for leadership.

15. Develop presentations, diagrams, and flowcharts for clinical staff to follow.

16. Support the EPIC electronic medical record development and implementation.

17. Attend scheduled department and division meetings and participate as appropriate.

QUALIFICATIONS:

  • Associate’s degree required, bachelor’s degree preferred. 
  • 2 year’s experience in healthcare quality settings with data or statistical analysis, data manipulation, and reporting is preferred.
  • Candidates must have excellent advanced skills with Microsoft Excel, and be proficient at vlookup, pivot tables and formulas.
  • Excellent written and oral communication skills
  • Attention to detail and accuracy
  • Excellent organizational skills
  • Ability to juggle multiple tasks and timelines, to work in a fast-paced environment
  • Knowledge of managed care, population health, and value-based care preferred
  • Ability to do database management tasks
