Case Manager

We have an immediate need for Case Managers to join our team! As an SCES Case Manager you will assess individuals for their eligibility in a variety of programs that may be available to them, conduct home visits, and document your assessments. You will create care plans in collaboration with your team and will then coordinate and monitor those services on an ongoing basis, while acting as an advocate for those on your caseload. Because of the communities we serve, we encourage those with fluency in languages in addition to English to apply.

Requirements of Applicants:

Although applicants with Bachelor’s Degree are preferred, those who have experience working with elders and have earned an Associate’s Degree will be considered as well as those who have completed 50% of their coursework required for a B.A. with a plan to complete the remainder within three years.

Reliable transportation, current driver’s license and clean driving record are required.
