What You'll Do:
- Provide rehabilitation and exercise-based care under the direction and supervision of a licensed Physical Therapist.
- Implement individualized treatment plans established by the Physical Therapist.
- Monitor patient progress, communicate changes in patient status, and assist in modifying treatment plans as directed.
- Educate patients and their families on home exercise programs, injury prevention, and strategies to support long-term success.
- Document patient treatments, progress, and outcomes accurately and in compliance with state and federal regulations.
- Collaborate with Physical Therapists and the clinical team to deliver exceptional patient experiences and achieve outstanding clinical outcomes.
Qualifications
- Graduate of an accredited Physical Therapist Assistant (PTA) program.
- Current Connecticut Physical Therapist Assistant license in good standing (or eligible at the time of hire).
- Current CPR/BLS certification (American Heart Association or equivalent).
- Proficiency with electronic medical records (EMR), preferably Prompt EMR or equivalent.
- Strong documentation skills compliant with CMS and Connecticut state regulations.
- Ability to perform the physical demands of patient care, including standing, lifting, assisting, and transferring patients.
- Excellent communication and interpersonal skills with patients, families, and interdisciplinary team members.
