NORC Case Manager
Make a meaningful, direct impact in your community! As a Case Manager for our Naturally Occurring Retirement Community (NORC) program, you will empower older adults to maintain their independence, dignity, and quality of life as they age in place.
Working under the guidance of the Program Director, you will serve as a trusted advocate, navigator, and supportive guide for local seniors—helping them access vital resources, manage challenges, and stay connected.
What You’ll Do (Key Responsibilities)
Client Engagement & Advocacy: Build warm, trusting relationships with older adults in their homes, the NORC office, and community settings to understand their goals, strengths, and unique needs.
Comprehensive Assessment & Care Planning: Conduct multidimensional psychosocial assessments, health indicators surveys, and wellness screenings to create customized care plans with clients.
Service Coordination & Counseling: Connect clients with essential benefits, entitlements, and community resources; provide supportive casework counseling and crisis intervention when needed.
Health Promotion & Outreach: Spread the word about weekly NORC wellness events, workshops, and activities through outreach, promotional flyers, and community networking.
Ongoing Support & Evaluation: Maintain regular touchpoints with clients to monitor progress, adapt care plans, and ensure long-term well-being and service effectiveness.
Team Collaboration: Participate in bi-weekly case conferences and team meetings to share insights, update the Program Director, and refine care strategies.
Program Operations & Documentation: Maintain detailed client case notes in the VIVE electronic database, track monthly logs, welcome office visitors, and adhere to NYC Department for the Aging policies and privacy standards.
What We’re Looking For
Education: Minimum BA or BSW required (Social Work or human services field preferred).
Language: Bilingual fluency in English and Chinese (Cantonese or Mandarin) is required to effectively serve our community members.
Experience: Prior casework experience with older adults is strongly preferred.
