Patient Navigator

Job Title: Patient Navigator

Location: In-Person - Chicago, IL 

Reports To: Patient Navigator Supervisor

Department: Community Outreach and Engagement 


 

Compensation Range: 17.00 to 21.00

FLSA Status: Non-exempt

Resource Retrieval, Resource Navigation, and Resource Repository Maintenance

  • Identifying and maintaining an up-to-date repository of nonprofit, government, and managed care organization resources that help enrolled members with overcoming barriers to preventive care including insurance benefits, medication access, education, transportation
  • Ensuring the repository of resources used to support the CES team is well-maintained and understood by all team members, and actively participating in trainings to share knowledge, highlight available benefits, and promote effective resource utilization.
  • Working with enrolled patients to provide them with the resources in the repository based on the individual needs identified and discussed with the member and the member’s PHCC
  • Ensure that members understand the directions required for the social need support and any requirements for accessing the referred service, tailor communication to appropriate health literacy levels
  • Coordinate referrals for the basic determinants of health (e.g., food, clothing, shelter, income, utilities) 
  • Partner with supervisors to gain exposure to skills required to optimal resource retrieval and navigation assess patient readiness for change to achieve MPCEO outcomes
  • Understand real-time availability/capacity of community-based resources and contribute to actively manage appropriate access, engagement, follow-up, and coordination of services
  • Excellent communication skills and understanding of the MPCEO core mission and the importance of preventive health; high level and compelling ability to message how MPCEO enrollment can add value to members facing barriers to health care
  • Execute the documentation of all Patient Navigation services in the electronic health record-based forms in an online digital platform  
  • Proficiency in cross-training activities including assisting patients with patient-facing components of the EHR Module and video visit function, assisting any enrolled patients with their preventive health journey by re-enforcing the clinical care plan outlined by preventive and behavioral health clinicians and by addressing any barriers to healthcare associated with social determinants of health or unmet social needs
  • Communicate clearly and effectively to team members, clinicians, and patients
  • Comfort with regular and routine documentation of all duties for MPCEO programmatic and evaluative purposes
  • Participate in weekly team meetings, daily team huddles, and supportive supervision
  • Perform all job functions in compliance with applicable federal, state, local and company policies and procedures
  • Knowledge and compliance with all HIPAA policies, procedures, and laws
  • Allow new employees participating in training programs to shadow you, and provide coaching and support to new peers who have joined the team, as directed by management and as needed to support business performance.


 

Qualifications

REQUIRED EDUCATION and/or EXPERIENCE 
We welcome candidates with a variety of educational and professional backgrounds that demonstrate the skills and experience needed to succeed in this role.

  • High school diploma required
  • High-level communicator with a growth mindset; fast learner 
  • Energized by the challenge of resource retrieval and resource navigation for Medicaid members
  • Experience with working in high-performance setting driven by productivity metrics
  • Any combination of 1 years public health, social services, client services experience preferably in health care setting or community base organization
  • Excellent time management and organizational skills
  • Demonstrated ability to work independently and in teams
  • Positive attitude, ability to show empathy and compassion
  • Proficiency in completing all patient documentation in an electronic medical record 
  • Proficiency in sending and receiving staff messages to/from care team members in an electronic health record
  • Compliance with all our Human Resources Policies and Procedures related to job title 
  • Driver’s License with verifiable good driving record and reliable transportation

This role is an in-person position and requires full-time on-site attendance in a call center in Chicago, IL.
