Intern/Personal Trainer
CLIENT COMMUNICATION
Once a Client is assigned to and engaged with the Contractor, Freeform Fitness LLC will not be in direct communication with the Client without notifying the Contractor. Any communication made to Freeform Fitness LLC by a Client engaged with a Contractor, will be documented and sent as accurately as possible to the Contractor. However, it is generally understood that any significant issue (positive or negative) should be communicated immediately by the Contractor to Freeform Fitness LLC. Freeform Fitness LLC does reserve the right to periodically check in on the Client’s progress but will notify the Contractor prior to doing so.
Contractor is responsible for responding to all Client communication within 12 hours with transparency and courtesy.
PERSONAL TRAINER CERTIFICATION
Contractor hereby certifies that they are qualified to perform the services of a Personal Trainer. Contractor will submit their valid Personal Trainer Certification along with this signed agreement.
INSURANCE
Contractor hereby certifies they have both general and professional liability coverage. A comprehensive policy should cover the Freeform Fitness LLC as well as the Contractor.
PROGRAMMING
Contractor is responsible for all Client Session Programming. Programming will occur prior to all scheduled Client Sessions. Programming will be available to Freeform Fitness LLC to review via Google or another form of electronic communication.
PROFESSIONALISM
Contractor is responsible for training, supervising , evaluating, scheduling, and any other requirements for all Clients assigned to them. Contractor warrants that they will be available, on time and appropriately dressed for all Client Sessions. If the Contractor is unable to make a scheduled session or will arrive late, it must be communicated to the Client as early as possible.
ATTIRE/UNIFORM
Freeform Fitness LLC requires that the Contractor wear a Blue Shirt and Black Shorts or Pants. Freeform Fitness LLC will reimburse Contractor for any expenses associated with attire/uniform or will provide them
