Case Management Nurse
SUMMARY
The Case Manager provides assessments and discharge planning in coordination with the providers to patients and families to assist them and the health care team in coping with patient's illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders, and other financial or social needs.

DUTIES AND AREAS OF RESPONSIBILITY
- Provide individual and family counseling within the scope of skill level.
- Provide nutrition education, fitness assistance, chronic disease maintenance, etc.
- Complete relevant forms to assist patients in obtaining whatever additional resources needed (e.g. free medications, utility bill paid, clothing, etc.).
- Consult with primary care provider, and other health professionals as required.
- Make referrals to medical provider as necessary.
- Monitor medication compliance.
- Provide counseling sessions for parent, home visits when appropriate.
- Use resource linkage.
- Meet regularly with other health center staff.
- Provide written and verbal descriptions of activity reports, utilizing existing and future patient software.
- Coordinate outreach activities with CEO and implement outreach activities approved by CEO.
- Function in a supervisory role as a team leader or trainer to outside students, when necessary.
- Screen, identify, treat and manage substance abuse problems in patients and family members.
- May perform these functions independently or as part of a team.
- Maintain working knowledge of and liaison with community agencies and resources.
- Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.
- Maintain records and statistics in accordance with department, hospital, medical center and/or health system policies.
- Attend and participate in staff, committee, department, and other administrative meetings.
- Participate in department, hospital, medical center, and/or health system continuous quality improvement efforts.
- Participate in regular supervisory conferences; keep current with social work and health care developments and seek to increase further enhancement of job-related knowledge.

EDUCATION AND/OR EXPERIENCE
Graduate of an accredited school of nursing, college or university.
Must hold a current and active Texas RN license or LVN license with no restrictions.
Must be qualified in BLS techniques.
Case Management Certification or 12months of full-time case management experience with intentions on receiving certifications within 1 year preferred
Must have a working knowledge of OSHA Bloodborne Pathogen Standards and Risk Management.
Have a working knowledge of Microsoft Office products and familiarity with network systems.
