Position Overview
As a Remote Health Educator, you will provide virtual health education and wellness guidance to patients through scheduled phone and video appointments. You will help individuals better understand their health conditions, treatment plans, preventive care, and healthy lifestyle practices while encouraging long-term behavior change. You will collaborate with interdisciplinary care teams to ensure patients receive consistent, high-quality support and education.
This role is ideal for candidates who are passionate about patient education, preventive healthcare, and building positive relationships through compassionate communication.
Key Responsibilities
* Conduct virtual health education sessions via phone and secure video conferencing.
* Educate patients on chronic disease prevention and management.
* Explain treatment plans and health recommendations using clear, patient-friendly language.
* Promote healthy lifestyle behaviors, including nutrition, physical activity, stress management, and preventive care.
* Develop individualized education plans based on patient needs and goals.
* Assess patient understanding and identify barriers to successful health management.
* Document all patient interactions accurately in electronic health records.
* Collaborate with nurses, physicians, care coordinators, and other healthcare professionals.
* Monitor patient progress and provide ongoing education and encouragement.
* Connect patients with appropriate healthcare and community resources.
* Maintain patient confidentiality and comply with HIPAA regulations.
* Participate in team meetings, training sessions, and continuing education activities.
* Stay current on evidence-based health education practices and clinical guidelines.
* Contribute to quality improvement initiatives that enhance the patient experience.
Qualifications
* Bachelor’s degree in health education, Public Health, Health Promotion, Nursing, Nutrition, Exercise Science, or a related healthcare field.
* Certified Health Education Specialist (CHES) or equivalent certification is preferred but not required.
* Strong verbal and written communication skills.
* Excellent interpersonal and active listening abilities.
* Comfortable using video conferencing platforms and electronic health record systems.
* Strong organizational and time management skills.
* Ability to work independently in a remote environment.
* Basic computer proficiency, including Microsoft Office and Google Workspace.
* Passion for patient education and preventive healthcare.
* Ability to maintain professionalism, empathy, and confidentiality.
* Previous healthcare, wellness, or patient education experience is preferred but not required.
* Recent graduates with relevant education, internship, volunteer, or clinical experience are encouraged to apply.
