Salary: $50,000 per year + Performance Bonus

We are seeking an energetic, compassionate, and highly motivated Outreach Manager to become a key member of our senior management team. This individual will serve as the public face of our agency, developing referral relationships, supporting clients and families, strengthening our community presence, and helping grow our organization. Social Workers would be an ideal fit!
Position Summary
The Outreach Manager reports directly to the Managing Director and is responsible for developing strategic partnerships throughout the healthcare community, expanding referral sources, supporting client satisfaction, and assisting with operational needs. This position combines relationship-building, business development, community engagement, and occasional direct client support.

Primary Responsibilities
Community Outreach & Business Development
Build and maintain relationships with hospitals, rehabilitation centers, nursing homes, assisted living facilities, hospice organizations, senior centers, physicians' offices, and other healthcare providers.
Schedule and conduct in-person meetings with administrators, discharge planners, social workers, nurses, and case managers.
Represent Sudbury River Home Care professionally as our primary community ambassador.
Develop long-term referral partnerships that generate new private-pay, VA, and other client referrals.
Maintain ongoing communication with referral partners to ensure strong relationships and continued collaboration.
Referral Source Development
Develop relationships with referral organizations including:
A Place for Mom
Jewish Family Services
Catholic Charities
Online referral platforms
Community organizations
Senior advocacy groups
Other healthcare referral sources
Respond promptly to inquiries and convert qualified leads into satisfied clients.
Client & Family Relations
Build trusted relationships with clients and their families.
Conduct client visits and follow-up calls.
Help families navigate home care options.
Address concerns and ensure exceptional customer satisfaction.
Assist families during changes in health conditions or care needs.
Caregiver Support & Crisis Response
Provide caregiver backup when staffing emergencies arise.
Assist with client crisis situations.
Participate in after-hours phone rotation to help maintain our commitment to providing 24/7 support for clients and families.
Office & Administrative Responsibilities
Assist with operational activities including:
Care Plans
Care Notes
Scheduling support
Documentation
Event coordination
Community presentations
Educational seminars
Website testimonials
Blog writing
Video content development
Other administrative duties as assigned
Qualifications
The ideal candidate will possess:
Excellent communication and interpersonal skills.
Strong networking and relationship-building abilities.
Professional presentation and public speaking skills.
Sales, marketing, healthcare outreach, or community relations experience preferred.
Experience working with seniors, healthcare organizations, or home care agencies is highly desirable.
Strong organizational and time management skills.
Ability to work independently while collaborating with management.
Compassion for serving seniors, veterans, and their families.
Requirements
Valid driver's license.
Reliable personal vehicle.
Ability to travel throughout the local community for meetings and events.
Ability to work both in the office and in the field.
Willingness to occasionally assist caregivers during client emergencies.
Ability to participate in rotating on-call phone coverage.
Compensation & Benefits
Annual Salary: $55,000
Paid bi-weekly through direct deposit
Annual performance bonus based on client acquisition, company performance, and individual contribution
Ten (10) paid vacation/personal days
Accrued paid sick leave

Company credit card for approved business expenses (including mileage and gas)
Interest-free employee loans of up to $2,000 (up to two-month repayment period)
Complimentary use of a company vehicle for up to two weeks, when available.
