Duties

• Assess patients’ physical conditions by reviewing medical histories and conducting comprehensive evaluations based on anatomy, physiology, and medical terminology.
• Develop tailored treatment plans utilizing techniques such as trigger point therapy, orthopedics, neurology interventions, and outpatient rehabilitation strategies.
• Implement therapy sessions focused on patient rehabilitation, mobility improvement, pain management, and functional independence enhancement.
• Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions across inpatient, outpatient, home health, and community care settings.
• Maintain accurate clinical documentation standards within electronic health records (EHR) systems and EMR systems to track progress and support compliance.
• Educate patients and their families on treatment procedures, home exercises, injury prevention, and overall wellness strategies.
• Conduct ongoing assessments to monitor progress and adjust treatment plans accordingly while adhering to clinical documentation standards.
