Notre Dame Mission Volunteers develops leaders through education and compassionate service that sparks confidence to learn and agency to transform disenfranchised communities. NDMV Volunteers provide direct in-person service at schools, community centers and non-profits. The Care Coordinator will help clients navigate healthcare access.

Responsibilities may include: Support the delivery of patient-centered care by assisting with volunteer-based care coordination activities, including referrals and follow up services; ensure patients receive secondary care and support needed while providing mentorship and assistance within the volunteer program; and connecting patients with resources that support whole person health.

Minimum Requirements

Applicants must be a U.S. citizen, national or lawful permanent resident at least 18 or older and have a high school diploma or GED. Applicants should have an interest in working with youth in an educational setting.
