Physician Assistant or Nurse Practitioner Contractor

Position Overview
We are seeking a compassionate and collaborative Nurse Practitioner or Physician Assistant to join our team as a contractor. This role is ideal for a provider who is interested in leading small, structured group medical visits or clinics for patients with substance use and mental health needs. The provider will work closely with a therapist and support staff to deliver coordinated, patient-centered care.
Key Responsibilities
- Provide assessment, medication management, and follow-up for patients with substance use and co-occurring mental health conditions
- Practice within a harm reduction framework aligned with the philosophy of the clinic
- Collaborate with a therapist and support staff to ensure smooth patient flow and coordinated care
Qualifications
- Licensed Nurse Practitioner or Physician Assistant
- Credentialed in Vermont or eligible for credentialing
- Experience treating substance use disorder and/or mental health conditions preferred
- Commitment to harm reduction and patient-centered care
- Comfortable working in a collaborative, group-based care model
- Experience with telehealth or virtual care is a plus
