The Chautauqua County Department of Mental Hygiene and Social Services is seeking a dedicated and compassionate Registered Professional Nurse II (Public Health) to provide professional nursing services to individuals and families throughout Chautauqua County.
This position offers an opportunity to make a meaningful impact in the community by delivering public health and behavioral health services in clinic settings, community locations, and client homes. The successful candidate will work collaboratively with physicians, nurse practitioners, physician assistants, mental health professionals, and other healthcare providers to promote the health and well-being of county residents.
Key Responsibilities
- Provide professional nursing care to individuals throughout Chautauqua County.
- Assist physicians, nurse practitioners, and physician assistants in mental health clinics.
- Conduct health assessments, obtain health histories, and perform physical evaluations.
- Manage medications and medical supplies, including ordering, inventory, storage, and security.
- Administer medications, immunizations, and contraceptives in accordance with standing orders and New York State guidelines.
- Counsel clients regarding health habits, family planning, reproductive health, sexually transmitted disease prevention, and healthy lifestyles.
- Perform pregnancy testing, provide options counseling, and coordinate referrals.
- Assess developmental status of infants and toddlers.
- Develop care plans and coordinate services with healthcare providers and community agencies.
- Conduct social and medical evaluations related to long-term care and home-based services.
- Provide case management, follow-up services, and health education to clients and families.
- Maintain accurate medical records, documentation, reports, and insurance-related records.
- Collaborate with Department forensic staff and multidisciplinary treatment teams.
- Utilize electronic medical record systems for documentation and case management.
- May assign tasks to lower-level caregivers as appropriate.
Minimum Qualifications
- Possession of a valid license issued by the New York State Education Department to practice as a Registered Professional Nurse (RN) at the time of application, appointment, and throughout employment.
- Applicants must attach a copy of their current RN license when applying.
Special Requirement
- Ability to meet transportation requirements necessary to perform field work assignments throughout Chautauqua County.
Note: Candidates holding nursing credentials from states with New York reciprocity agreements may be conditionally approved pending proof of New York State licensure.

- Strong clinical assessment and patient education skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent communication, counseling, and case management abilities.
- Experience with public health, behavioral health, community health, or outpatient clinical services is desirable.
- Proficiency with electronic medical records and documentation systems.
