Case Management Assistant
Job Summary
Providence In-Home Care Service is seeking a dedicated Case Management Assistant to provide essential administrative and client support to the Case Management team. This role plays a key part in coordinating client care services, maintaining accurate records, supporting communication between clients and service providers, and ensuring smooth day-to-day case management operations.
The ideal candidate is organized, detail-oriented, and committed to supporting high-quality in-home care services for clients in need of assistance with daily living, healthcare coordination, and community resources.
Key Responsibilities
- Support Case Managers in coordinating client care plans, services, and follow-up activities.
- Maintain accurate and up-to-date client records in electronic and paper filing systems.
- Prepare, organize, and manage case documentation, reports, and correspondence.
- Communicate professionally with clients, family members, healthcare providers, and community agencies.
- Schedule client appointments, assessments, and internal team meetings.
- Track referrals, service authorizations, and ongoing case progress.
- Enter and update client data in case management software and databases.
- Assist Case Managers in identifying and connecting clients with appropriate community resources.
- Ensure compliance with confidentiality standards, HIPAA regulations, and company policies.
- Support audits, reporting requirements, and quality assurance processes.
- Perform additional administrative duties as assigned by management.
Qualifications
Required
- High school diploma or equivalent (Associate degree preferred).
- Minimum 1 year of experience in administrative support, healthcare, home care, or social services.
- Strong organizational and multitasking abilities.
- Excellent written and verbal communication skills.
- Proficiency in Microsoft Office (Word, Excel, Outlook) and data entry systems.
- Ability to handle sensitive information with professionalism and confidentiality.
Preferred
- Prior experience in home care, case management, or healthcare coordination.
- Knowledge of community resources and social service programs.
- Bilingual abilities are a plus.
