Part-Time / Per Diem COTA, OTR, PTA, RPT – Home Health (Flexible Schedule)

Seraphic Springs Health Care Agency is seeking compassionate, motivated Physical Therapists (PT), PTAs, Occupational Therapists (OT), and COTAs to join our growing, clinician-led teams in the Milford/Webster and Fitchburg regions.

We prioritize quality over quotas, giving clinicians the time and support needed to deliver meaningful, patient-centered care in the home.

Role Responsibilities

  • Provide skilled therapy services in patients’ homes
  • Perform evaluations, treatments, and patient education
  • Collaborate with the interdisciplinary care team
  • Complete timely, accurate EMR documentation
  • Maintain compliance with home health regulations

Qualifications

  • Active Massachusetts license (PT, PTA, OT, or COTA)
  • Home health experience preferred
  • Strong communication and documentation skills
  • Reliable transportation and valid driver’s license
  • Ability to work independently with a flexible caseload
