Harborview Post-Acute is a skilled nursing and rehabilitation facility located in Memphis, Tennessee, with a capacity for 103 residents. It offers both private and semi-private suites and features amenities such as comfortable common areas, individualized dietary plans, laundry services, and religious services. The facility focuses on providing social and emotional support through its activities and social services programs, ensuring an engaging and supportive environment for its residents.
RESPONSIBILITIES
- Conduct comprehensive assessments to evaluate patients' speech and language abilities.
- Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.
- Provide therapeutic interventions, including but not limited to, articulation therapy, language development therapy, voice therapy, and cognitive training.
- Monitor and document patients' progress and make adjustments to treatment plans, as necessary.
- Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.
- Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
- Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.
QUALIFICATIONS
- New passer/graduates and CF supervision are welcome.
- Previous experience as Speech Language Pathologist (SLP) or Speech Therapist (ST) is a plus.
- With Skilled Nursing or Long-Term Care experience is a plus.
- Valid state licensure as a Speech Language Pathologist.
- Active CPR Certification.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP).
- Is in good standing with all regulatory agencies and licensing boards is required.
