Physical Therapist — Mobile Outpatient (W-2)
What we do
Jacob Rehab is a mobile outpatient physical therapy practice providing Medicare Part B services directly in patients' homes throughout Westchester County.
This is not home health. This is outpatient orthopedics delivered where patients live — without the limitations of a traditional clinic setting. Every session is 1:1, individualized, and built around real clinical reasoning and functional outcomes. We are not a high-volume, cookie-cutter exercise model.
Why this practice exists
This practice was built by a physical therapist who has spent over 15 years working within the New York healthcare system. Too often, clinicians are overworked, micromanaged, and forced into systems that prioritize volume over care. This model was created to be the opposite of that. Strong clinicians should be trusted — not controlled.
What the role looks like
- 1:1 outpatient physical therapy in patients' homes
- Medicare Part B — outpatient coding, not home health episodic care
- 25–30 visits per week (roughly half the volume of a traditional clinic)
- Patients geographically clustered to minimize drive time
- Flexible scheduling built around your autonomy
- Full administrative support — scheduling, billing, documentation systems, physician communication, and documentation training provided
Compensation & benefits
- Base salary: $92,000 (W-2 annual)
- Mileage reimbursement ($0.67/mile, tax-free)
- Health benefit (QSEHRA)
- Monthly stipend, tax-free
Total compensation — including mileage reimbursement and health benefit — exceeds $100,000 for a full schedule.
What we expect
- DPT from an accredited program — new graduates encouraged to apply
- Active NY state PT license, or eligibility to obtain prior to start
- Valid driver's license and reliable vehicle
- Strong clinical reasoning and genuine interest in outpatient orthopedics
- Ability to work independently and manage your own schedule
- Timely, accurate documentation — training on Medicare Part B coding provided
