Project Facilitator - Part time - M - T 9a-5p and Wednesday 9a-1p


Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary:
We have an exciting opportunity to join our team as a Project Facilitator - Part time - M - T 9a-5p and Wednesday 9a-1p.

In this role, the successful candidate The Family Health Centers at NYU Langones mission is to improve the health of underserved communities by delivering high-quality, culturally-competent health care and human services. Recognizing that health depends upon an individuals economic, educational and employment status, FHCs Department of Community Based Programs offers an extensive continuum of family strengthening and community development programs to address the broader needs of the community, including adult education; case management and supportive services; mental health services; early childhood centers; and community service opportunities.
Family Health Centers at NYU Langone's Department of Community Programs is seeking an exceptionally qualified candidate for the role of Project Facilitator. This position will be responsible for the implementation of family strengthening programs (for example, Reach Out and Read, Vroom) into clinical practices and community settings. These programs support the strengthening of the parent-child relationship as well as parents self-efficacy to support their childs early brain development (birth to five years old). Strategies for program implementation and integration include: outreach and recruitment, working with internal and external partners, training of staff, and providing tools and resources to families and stakeholders, and curating developmentally appropriate books and brain building tips.


Job Responsibilities:



Develop and implement clear work plans for program integration and expansion with defined benchmarks and timeline  


Ensure consistency of best practices for family engagement and support across program modalities. 


Develop and implement effective outreach and engagement strategies to enroll families in programming. 


Identify and engage key stakeholders from the various clinical and community settings for program integration 


Develop and implement strategies for scaling up programs within clinical and community settings 


Model developmentally appropriate activities for families, including reading and Vroom implementation 


Participate in and/or initiate community events and workshops to engage families in programming  


Ensure that implementation sites have consistent access to program resources including books. 


Ensure that implementation settings offer developmentally appropriate and engaging environments for children and families  


Implement interdisciplinary training and ongoing support for clinical and community-based program staff  




Establish and maintain efficient systems to monitor and track program deliverables to ensure fidelity to the program models 


Assist in the preparation of program grant proposals and reports 


Collect comprehensive, accurate data (including surveying, focus groups, distribution of materials) to document and demonstrate program effectiveness. 


Assists in data analysis for continuous program improvement 


Train and provide onsite structure and support for volunteers focused on program implementation in healthcare settings 


Attend staff meetings and relevant trainings


Other related duties as assigned by program leadership 



Minimum Qualifications:
To qualify you must have a B.A.
Experience in community engagement or other forms of direct outreach
Strong leadership, interpersonal, and relationship management skills
Excellent communication, project management, and multi-tasking skills
Experience with data management and reporting
Proficiency in use of technology such as laptop computers and tablets


Preferred Qualifications:
Demonstrated cultural competence with respect to Sunset Park, Brooklyn
Understanding of New York Citys diversity and experience working with people of diverse backgrounds

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $21.94 - $34.89 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
