CPST Case Managers provide an array of services in the community to clients with severe and persistent mental illness. It is necessary to be mobile, work with multidisciplinary teams to assist the client in meeting their recovery goals as outlined in treatment planning. CPST’s assess client’s needs and aid in linking to community-based resources. That may include areas of food, shelter, employment and various other community resources. Services may be face-to-face or by telephone. Services may be provided individually or in groups. Services are not site specific but must be in locations that meet the needs of the people served.
