CPST Specialist

Far West Center is an award-winning community mental health center with locations in Westlake (Cuyahoga County) and Amherst (Lorain County), Ohio. We are accredited by The Joint Commission and certified by the State of Ohio to provide outpatient mental health services for individuals and families. CPST SPECIALIST

 

Position Summary: The Community Psychiatric Supportive Treatment Specialist (Case Manager) position assists adults with severe mental illness in their recovery and helps maintain stable, healthy living in the community.  Positions available at Westlake and Amherst offices.

Responsibilities:

  1. Provides direct client CPST(case management) services, outreach and support. 
  2. Supports client treatment and service goals through individual face-to-face and telephone contacts.
  3. Accesses entitlements and benefits on behalf of clients.
  4. Maintains updated information about community resources, and links clients to resources for their health and well-being.  Liaison with other agencies on behalf of clients.
  5. Assures compliance with treatment plans, and assists clients in managing symptoms and practicing good mental health practices.
  6. Supports clients in their recovery process. 
  7. May provide services individually or in a group setting.
  8. Meets productivity and quality of care standards for services. Completes all documentation accurately and timely.  
  9. Actively participates in supervision and team meetings as required.
  10. Contributes information to the treatment team to assure optimal care for clients.  

 Requirements:

  1. Bachelor’s Degree in Social Work, Counseling or related Human Service field required. LSW or LPC highly preferred.
  2. One year (1) of recent, related experience in mental health field following graduation and post-licensure.
  3. Current knowledge of community resources and public entitlements.
  4. PC Proficiency required.
  5. Excellent written and oral communication and problem-solving skills.
  6. High quality organization and timely documentation skills.
  7. Reliable transportation, automobile insurance and good driving record required.
