Truancy Case Manager

Positions available at Northeast and West Philadelphia location.

Family Related Duties

  • Making initial contact within 8 hours of receiving a referral and immediately documenting contact within the CBPS database.
  • Making physical contact with the youth (at school) or family (at home) within 24 hours of case assignment.
  • Conducting attendance assessments of all school aged children in the home and discussing the Impact of Truancy Information Sheet at the initial home visit. Completing the Family Development Plan within 30 days of program enrollment/intake and updating it every 90 days.

School Related Duties

  • Providing monthly school visits or as needed to gather the most up-to-date information regarding attendance, specialized services, supports and academic performance data.
  • Assuring that students on caseload are enrolled in summer enrichment or education programming during the summer months.
  • Actively collaborating with the School District in resolving all school-related barriers affecting a student’s attendance including teachers, counselors, psychologists, School Based Social Service providers etc.
  • Documenting and monitoring efforts made by the School District to meet the identified educational needs of the student: IEP requirements, learning supports, issues related to bullying, etc.

Requirements

  • Bachelor’s Degree in Social Work or related field and 2 years of verifiable experience with high risk youth and families in similar settings
  • Must be willing and able to work flexible hours to increase engagement with difficult to contact, transient youth and families identified for the service
  • Must be willing to travel throughout Philadelphia and conduct home and school visits
