Promise Care Manager
About the Opportunity
Code Differently is seeking a compassionate, organized, and community-focused Promise Care Manager to support the State of Delaware’s PROMISE Program in Newark, Delaware. This temporary position plays a critical role in supporting individuals with severe and persistent mental illness, including individuals with co-occurring disorders.
The Promise Care Manager will work as part of an interdisciplinary care team to help individuals access appropriate services, develop individualized recovery plans, and ensure continuity of care through collaboration with providers, community organizations, and support networks.
This role is ideal for professionals passionate about behavioral health, case management, advocacy, and improving outcomes for vulnerable populations.
Key Responsibilities
- Conduct comprehensive client assessments and evaluations
- Assist in developing individualized recovery and treatment plans
- Determine appropriate levels of care and coordinate placement into services
- Monitor and ensure delivery of care according to established standards and guidelines
- Maintain accurate case documentation, charting, and reporting
- Conduct community visits and engage with beneficiaries in various settings
- Serve as a liaison between clients, families, healthcare providers, and community organizations
- Coordinate referrals and connect clients to supportive resources and services
- Participate in interdisciplinary team meetings and case reviews
- Support clients in achieving recovery, stability, and wellness goals
- Maintain confidentiality and compliance with State and organizational policies
Qualifications
Required Qualifications
- Bachelor’s degree from an accredited college or university
- Valid driver’s license and reliable transportation
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively within a team environment
- Experience with case management, behavioral health, social services, or community outreach preferred
- Strong organizational and documentation skills
Preferred Qualifications
- Experience working with individuals experiencing mental illness or co-occurring disorders
- Knowledge of community-based behavioral health resources
- Familiarity with care coordination and recovery planning

Pay: $23.50 per hour
