Advanced Practice Provider (PA, NP)

The Rockbridge Area Health Center (RAHC) seeks an enthusiastic and dedicated Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to provide a full range of primary care and preventive medical services with a focus on pediatric patients. Supports access to comprehensive clinical services, including integrated primary medical, preventive, dental, and mental health care and a diverse array of health education and health promotion services.
 

 

QUALIFICATIONS
 

 

    EDUCATION, CREDENTIALS, AND EXPERIENCE
-    Current license to practice as an NP/PA in the Commonwealth of Virginia.
-    BLS (basic life support)/CPR (Cardiopulmonary resuscitation) required.
-    At least 1 year experience.
 

 

    KNOWLEDGE, SKILLS, AND ABILITIES
-    Ability to effectively communicate and collaborate with employees in cross-functional areas in a variety of roles to ensure exceptional patient care and efficient adherence to Standard Operating Procedures.
-    Proficiency using windows and cloud based clinical systems for coordination of patient care.
 

 

    FITNESS FOR DUTY
-    Must meet employee health requirements as detailed in RAHC’s clinical Fitness for Duty Policy.

 

ESSENTIAL FUNCTIONS
 

 

-    PROVIDES COMPREHENSIVE HEALTH CARE SERVICES TO ALL PATIENTS, INCLUDING BUT NOT LIMITED TO, WELL-CHILD CARE, ACUTE CARE AND CHRONIC CARE.
 

 

-    PROVIDES PATIENT AND FAMILY EDUCATION TO PROMOTE WELLNESS, TO PREVENT HEALTH PROBLEMS, AND TO MAINTAIN CURRENT HEALTH STATUS AS WELL AS INTERVENE IN ACUTE OR CHRONIC ILLNESS STATES.
 

 

-    PERFORM ROUTINE PHYSICAL EXAMINATIONS AND PREVENTIVE HEALTH MEASURES INCLUDING, BUT NOT LIMITED TO IMMUNIZATIONS, DEVELOPMENTAL SCREENING, ROUTINE LABORATORY SCREENING/PROCEDURES APPROPRIATE TO EACH GENDER AND AGE GROUP.
 

 

-    PROVIDES PATIENT EDUCATION OPPORTUNITIES FOR INDIVIDUALS AND/OR FAMILIES UTILIZING ADDITIONAL RESOURCES WHEN NECESSARY.
 

 

-    COORDINATES CLIENT CARE WITH OTHER COMMUNITY PROFESSIONALS AND ORGANIZATIONS FOR OPTIMAL CARE.
 

 

-    CONTRIBUTES TO THE HEALTH EDUCATION OF INDIVIDUALS AND GROUPS THROUGH OUTREACH PROGRAMS.
 

 

-    EVALUATES EFFECTIVENESS AND EFFICIENCY OF CARE AND PARTICIPATES IN QUALITY IMPROVEMENT ACTIVITIES AS A TEAM MEMBER TO IMPROVE THE VALUE OF CARE PROVIDED.
 

 

-    MAINTAINS CURRENT CONTINUING EDUCATIONAL HOURS, CERTIFICATION, AND LICENSURE.
