Advanced Nurse Practitioner (BHH Unit Program Leader)

The Southwest Connecticut Mental Health System (SWCMHS), under the umbrella of the State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking a motivated and mission-driven individual to join our team as an Advanced Nurse Practitioner (PCN 115156). In this role you will be a clinical leader and consultative advisor to support our clients within our Behavioral Health Home (BHH) care unit, across our three facilities.
 

POSITION HIGHLIGHTS:

  • Work Schedule: Monday - Friday
  • Work Hours: Full-time, 40 hours per week
  • Work Shift: First shift, 8:00 a.m. - 4:30 p.m.
  • Location: 1635 Central Ave, Bridgeport, CT (This role will also require occasional travel to our facility in Stamford)

 

WHAT WE CAN OFFER YOU:

 

WHAT YOU'LL BE DOING:

  • Provide Behavioral Health Home services to participants at all three SWCMHS sites (travel required).
  • Consult with treatment team and psychiatrist regarding specific Behavioral Health Home enrollee health issues.
  • Offer traditional medical care recommendations to mitigate morbidity rates within the high-risk population
  • Provide consultation with Behavioral Health Home Team to monitor implementation of the recovery plan to achieve integrated care outcomes consistent with recovery plan goals.
  • Assist in recovery plan development and coordination of services with other medical professionals.
  • Educate staff on BHH goals and core competencies.
  • Assist in enrolling individuals in BHH.
  • Teach and provide education on BHH services, documentation, and coding to staff and clients.
  • Teach and provide instruction of health condition management and prevention to staff and clients.
  • Facilitate psychoeducation groups congruent with the BHH initiative.
  • Attends and participates in treatment team meetings and recovery planning meetings.
  • Provide supervision to BHH nursing staff.
  • Document client contact, treatment, and treatment consultation in accordance with agency policies and Joint Commission standards.
  • Provide nursing coverage as needed
  • Complete BHH Consultant Tracker on a weekly basis

Please note:

 

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

A current license as an Advanced Practice Registered Nurse issued by the Connecticut Department of Public Health.

 

PREFERRED QUALIFICATIONS

  • Experience and ability to travel between multiple service locations to support client-related needs, including retrieving paper medical records and participating in meetings or program activities.
  • Bilingual English-Spanish speaking
  • Experience treating individuals with serious and persistent mental illness.
  • Experience providing health literate information to individuals in recovery.
  • Experience treating individuals with chronic medical conditions.
  • Experience treating individuals with serious and persistent mental illness.
  • Experience as a Family Nurse Practitioner

 

SPECIAL REQUIREMENTS

  • Incumbents in this class must possess and maintain Connecticut Controlled Substance Registration for Practitioners issued by the State of Connecticut Department of Consumer Protection.
  • Incumbents in this class must possess and maintain United States Drug Enforcement Administration Registration issued by the United States Federal Government.
  • Incumbents in this class must possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. 1320a-7b(f).
