School Based Services Case Manager


School Based Services Case Manager


Play a vital role in supporting children, youth, and families in your community. This position provides community-based assessment, treatment, and support for children and youth with severe emotional disturbances or developmental disabilities, along with their families. You'll help connect individuals to essential resources and care, making a meaningful impact every day.

 

Job Duties

  • Provide clinical and administrative oversight for up to 10 School-Based Interventionists, including weekly supervision, staff training, documentation review, and approval of timesheets and expenses.
  • Support staff development within school teams and broader school communities, including crisis response and on-call support outside regular business hours.
  • Establish and maintain service contracts with local schools and district partners.
  • Collaborate with multidisciplinary providers to develop individualized treatment plans and ensure coordinated care.
  • Facilitate treatment and educational team meetings and provide case consultation as needed.
  • Support children and youth with severe emotional disturbances and/or developmental disabilities by strengthening natural support networks and connecting families to community resources.
  • Assist families in accessing medical, social, educational, vocational, housing, and financial services.
  • Maintain accurate and timely electronic medical records, including treatment plans, contact notes, and required documentation.
     

Qualifications

 

Educational Requirements

  • Bachelor’s degree in human service or related field preferred.

 

Qualifications and Core Competencies

  • Two years of human service delivery with children and families preferred.
  • Experience providing direct instruction and therapeutic services to children with challenging behaviors preferred.
  • You must have a valid driver’s license, a clean driving record, and access to a safe, dependable, and insured vehicle.
  • The ability to lift and carry 50 pounds and execute physical restraints.
