Guardianship Case Manager

Position Summary:

Will be responsible for overseeing various types of clients including: 1. Guardianship, 2. Conservatorship, and 3. Fiduciary Management. The Case Manager will carry out responsibilities for fiscal management of client’s assets and act as an advocate for clients in all areas of their lives as appropriate. Per court, state and National Guardianship Association (NGA) standards, offers testimony and fulfills visitation contact and oversight. Interacts with necessary community agencies.


 

Duties And Responsibilities:

A. Guardianship Duties and Responsibilities

Attend annual Personal Center Plan (PCP) meetings of Riverwood clients and other related care meetings for WMG clients.

Keep the Court updated on changes to client’s information. 

Provide consent to treat authorizations as appropriate to medical providers. Give consent for medications as appropriate.

Provide case management support to link clients to services and to provide appropriate placements in the least restrictive environment for clients and families.

Maintain contact with client’s family members, as appropriate.


 

B. Conservatorship Duties and Responsibilities

Understand and manage client’s investments, annuities and certificates of deposit.

As Conservator, manage the maintenance and if necessary, the sale of property.

Maintain contact with client’s family members, as appropriate.


 

C. Fiduciary Management Duties and Responsibilities

Manage client’s Social Security and/or Veteran’s Administration (VA) benefits.

Create annual budgets for each client and update as necessary through the year.

Ensure client bills are paid in accordance with Social Security and VA guidelines.

Monitor client account spend downs to ensure continuation of governmental benefits and services.

Complete financial documents and/or provide financial information when requested.

Maintain contact with client’s family members, as appropriate. 


 

D. General Duties and Responsibilities

Complete annual Court/Social Security/VA reports on a timely basis.

Complete all agency and contractual paperwork on a timely basis, keep clients records updated and current, and provide all necessary paperwork required to prove that services are being provided.

Attend court hearings as required being prepared to offer testimony if necessary.

Follow contract and best practice guidelines for clients regarding home visitation and personal contact.

Monitor Medicaid re-determination and spend downs.

Ensure client funds are spent in a manner that is in the best interest of the client.

Advocate for client’s needs in the capacity assigned.  

Responsible for On-call phone as assigned by supervisor.

Serve on agency committees as assigned.

Will attend CPR and First Aid trainings and will keep certifications current.

Will attend all mandated trainings required and will keep certifications current.

Other duties as assigned.


 

Qualifications:

Ability to complete multiple duties and tasks correctly and on time without being prompted.

Ability to learn and use program specific computer applications.

Ability to work with team members and others to research, plan and implement initiatives to develop new and existing services.

Possess excellent communication, organizational and writing skills.

Ability to work with a diverse population of clients and community systems.

Knowledge of community resources.

Willingness to complete training regarding Michigan Probate laws.
