Supports Coordinator – Family Services/Children

Pay: $50,112 annually

Job Type: Full-Time | Exempt

About the Role

The Supports Coordinator serves as the primary advocate and resource broker for individuals and families. This role ensures access to services, entitlements, and supports while promoting Person-Centered and Family-Centered Planning principles. Responsibilities include developing and implementing Individual Plans of Service (IPOS), coordinating care, and fostering community connections.

 

Key Responsibilities

  • Facilitate and implement IPOS based on individual needs and preferences
  • Link, coordinate, and monitor services outlined in the IPOS
  • Assist families with accessing entitlements (Medicaid, SSI, Children’s Waiver, Autism Benefit, etc.)
  • Support crisis planning and resolution
  • Coordinate health and safety strategies, including discharge planning
  • Participate in school transition planning and IEP meetings
  • Promote community inclusion, self-advocacy, and relationship building
  • Complete timely documentation and maintain compliance with agency and funder requirements
  • Participate in required trainings and professional development

 

Qualifications

  • Bachelor’s degree in Human Services or related field
  • Minimum one year of experience supporting individuals with I/DD; experience with foster care/adoption preferred
  • Meets requirements for provisional QIDP credential; full QIDP preferred
  • Strong communication, organizational, and computer skills
  • Valid Michigan driver’s license, insurance, and acceptable driving record

 

Additional Requirements

  • Extensive travel within Wayne County; reliable transportation required
  • Completion of all required trainings, including QIDP and Children’s Waiver-specific training
  • Must pass criminal background, sex offender registry, and protective services clearance
