Early Intervention Service Coordinator
Assists parents of eligible infants and toddlers in gaining access to the Early Intervention services and other services identified in the IFSP, according to Article 25 Early Intervention law and regulations. Coordinates and monitors the provision of EI services.
Understands the use of technology in the EI program. Uses the NYEIS system efficiently and accurately.
Ensures the IFSP outcomes and strategies reflect the family’s priorities, concerns and resources and that changes are made as the family’s priorities, concerns and resources change.
Facilitates the development of a transition plan to preschool services, if appropriate, or to other available supports and services.
Maintains necessary documentation, in a timely and accurate manner, required in child’s record and for reports to the State.
A) Graduation from a regionally accredited or New York State registered college or university with a Bachelor's degree in Education, Psychology, Sociology, Nursing, or closely related field; OR
B) Graduation from a regionally accredited or New York State registered college or university with a Bachelor’s Degree and one year of experience in case management, nursing, social work, or teaching: OR
C) An equivalent combination of training and experience as defined by the limits of A) and B) above.
