Case Manager
Position Overview
Kaleb House is seeking a compassionate and organized Case Manager to support survivors participating in the Kaleb House restoration program. The Case Manager will serve as a primary point of coordination for survivor services, helping clients navigate housing stability, healthcare, legal processes, education, employment, and long-term recovery goals.
The Case Manager plays a critical role in ensuring survivors receive consistent, structured support while progressing through the Kaleb House program. This position works closely with counselors, medical staff, and program leadership as part of a multidisciplinary care team dedicated to holistic restoration.
Preference will be given to candidates with experience working with the Oklahoma Department of Human Services (DHS) or similar agencies that provide services to survivors of abuse, trafficking, or vulnerable populations.
Primary Responsibilities
Client Case Management
- Conduct initial intake assessments and help develop individualized service plans for survivors entering the program.
- Track client progress and adjust service plans as recovery needs evolve.
- Assist survivors in accessing resources including healthcare, counseling, legal support, identification documents, education, and employment opportunities.
- Maintain organized and confidential case records in accordance with program policies.
Survivor Advocacy
- Advocate for survivors within community systems including healthcare providers, legal services, and social service agencies.
- Assist survivors with navigating complex systems such as benefits enrollment, court processes, and victim services programs.
- Support survivors in developing practical life skills and personal independence.
Off-Site Survivor Support
- Provide two days per week of off-site survivor support, which may include:
- Transportation and accompaniment to appointments
- Coordination with external service providers
- Assistance with housing, employment, or education opportunities
- Support during legal or advocacy-related appointments
Multidisciplinary Team Collaboration
- Work closely with the Licensed Professional Counselor, Registered Nurse, and program leadership to ensure coordinated care for each survivor.
- Participate in case review meetings and contribute updates on client progress.
- Identify barriers to stability and coordinate solutions with the care team.
Documentation & Reporting
- Maintain accurate and confidential case documentation.
- Assist with internal reporting requirements and program outcome tracking.
- Ensure compliance with organizational policies related to confidentiality and client rights.
Preferred Qualifications
- Bachelor’s degree in social work, psychology, criminal justice, or a related human services field.
- Experience working with survivors of human trafficking, domestic violence, or abuse strongly preferred.
- Preference given to candidates with experience working with the Oklahoma Department of Human Services (DHS) or similar survivor support programs.
- Knowledge of community resources, victim advocacy programs, and trauma-informed care practices.
- Strong organizational and documentation skills.

Work Schedule
- 1x 24-hour on-site shift per week with private residential accommodations provided for sleep and food preparation.
- 2 days per week of off-site survivor support and service coordination.
- 1 week out of every 4 weeks serving in an on-call rotation to assist with client needs or urgent case management issues if needed.
Compensation
This position is salary-only compensation with no additional benefits or incentives.
$50-55,000 salary based on experience and employment types.
