Case Manager
Case Manager with Transitions Thru Life
This position requires an individual who practices consistent high-quality case management, education, advocacy, and referrals that coordinate linkage with appropriate community service providers/agencies per client needs. This is accomplished in collaboration with other members of the multidisciplinary treatment teams, courts, and local agencies as appropriate.
Duties:
Screening/Assessment - The Case Manager assess clients' strengths, limitations, and supports. They target service needs to maintain the individual and family in their home safely. The Case Manager will work with the MDT to develop a treatment plan based on assessments and service recommendations with the client and family. Initial assessments will be completed within 14 calendar days. Case Manager will contact the client within 48 hours of receiving the referral.
Treatment Planning/Goal Setting - Case Manager will monitor the implementation of services and progress toward treatment goals. Case Manager will attend court sessions and MDT as required by case needs.
Referral to service providers - Case Manager will maintain a guide to community resources after assessing the client's needs. The case manager is responsible for educating the client, caregiver, and immediate family members on available services and linking them with the appropriate referrals based on the client’s identified needs.
Advocacy - Case Manager will demonstrate competency and an understanding of CPS and court protocols. Case Manager will act as an advocate in the best interest of the client/family. When appropriate, Case Manager will educate and encourage the family to advocate on their own behalf, fostering independence and empowerment.
Crisis Intervention - Case Manager workers can make rapid assessments and develop crisis management plans to maintain clients in the home. Case Manager will access multiple resource directories to meet the demands of a crisis. In the event of a medical or psychiatric emergency, staff will follow the protocols of the courts, DHHR, & T.T.L, inc.
Work Hours are flexible. Clinicians may set their schedule on an individual basis.
Remote: This position requires the applicant to be able to work in the community and from home.
Travel: Required 25-50%

- No prior CPS/APS history
Knowledge, Skills, and Abilities: Candidates should be comfortable with the following: (a) Knowledge of community resources, how to make appropriate referrals to community/governmental agencies for services, and the ability to coordinate services. (b) Skill in independently conduct assessments and provide treatment interventions to a wide variety of individuals from diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures are needed. (d) Skill in independently implementing different treatment modalities in working with individuals and families experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to workers and staff about the psychosocial needs of clients and the impact on care and compliance with treatment.
Education: Bachelor's degree in social work (or related field) from a school fully accredited by the Council on Social Work Education (CSWE) A transcript or degree must be submitted with your application. Note: Degrees recognized by the U.S. Department of Education from accredited colleges, universities, schools, or institutions may be used to qualify as a Case Manager. Verify your education by uploading a copy of your degree. A master’s degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
