Key Responsibilities:

• Conduct assessments, develop care plans, and track progress
• Coordinate medical, behavioral health, and social services
• Educate and support clients and families
• Advocate for high-quality, coordinated care
• Maintain accurate documentation
• Participate in team meetings and case conferences

Qualifications:

• Bachelor's in social work, nursing, psychology, or related field
• Experience with complex medical and behavioral health needs
• Strong communication and teamwork skills
• Proficiency in Microsoft Office & electronic health records
• Bilingual skills a plus
