Description

Contributes to the development, implementation, monitoring, and updating of Consumers’ individualized habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed.  Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements.  Other duties as assigned by management staff.   Must be able to operate an Agency vehicle and maintain a driving record acceptable to the Agency’s insurance provider.

 

This position is Full-Time, working  7:00 a.m. to 3:00 p.m.

Qualifications

Education/Experience:

Bachelor’s Degree from an accredited college or university in a helping profession, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.

Bachelor’s Degree in a field other than a helping profession must have a minimum of two years of professional experience in a community behavioral healthcare setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.

OR

Associate's Degree from an accredited college or university in a helping profession, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice plus a minimum of two years of professional experience in a community behavioral healthcare setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.
