Job Description

Position Title: Physical Therapist

Department: Rehabilitation Services

Grade: CN27 Status: Non-Exempt Date: May 2002

Reports To: Director, Supervisor, Senior Therapist, and Attending Physicians

 

 

Main Function

Evaluate, establish, and administer physical therapy treatment programs for patients based on physician referrals. Services may be provided at Noyes Memorial Hospital, Noyes Health Services, or both.

 

 

Key Responsibilities Clinical Duties

· Perform technical responsibilities accurately and in accordance with policy.

· Conduct initial evaluations/treatments for both inpatient and outpatient cases.

· Maintain documentation and individualized treatment plans in patient charts.

· Complete progress notes and discharge summaries per policy.

· Advance patients through treatment levels appropriate to their condition.

· Educate patients and families on home exercise programs and document accordingly.

· Record communications with medical, nursing, and social services staff.

Departmental Support

· Assist management in identifying departmental needs and setting goals.

· Perform additional duties as requested by the Senior Therapist or Director.

· Provide patients with vendor lists or referrals to Social Services and document accordingly.

· Demonstrate understanding of growth and development across the lifespan.

Customer Service & Teamwork

· Deliver services that meet or exceed expectations.

· Serve as a resource for hospital staff and provide at least one in-service annually.

· Respond to hospital emergency codes as assigned.

 

 

Qualifications Education & Experience

· Master’s or Doctoral degree from an accredited physical therapy program.

· Licensed Physical Therapist or eligible for licensure in New York State.

 

 

Work Environment Contacts

· Frequent interaction with departmental staff and patients.

· Regular collaboration with other departments and physicians.

· Daily lifting and physical strain associated with patient care.

Working Conditions

· Excellent overall working conditions.

Hazards

· Potential exposure to blood and bodily fluids during patient contact.

 

 

Patient Population

· Patients across the lifespan, from infancy to geriatrics.

· Emphasis on musculoskeletal and general medical diagnoses including CHF, COPD, diabetes, pneumonia, cardiac conditions, and Swing Bed patients.

 

 

Required Personal Protective Equipment

· Goggles

· Gloves

· Respiratory Face Mask

· Fall Protection Devices

· Insulation Gown (Plastic Apron)
