The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is currently seeking a qualified and experienced individual for the position of Professional Counselor to join our team.

  • UNIT: Assertive Community Treatment (ACT Team)
  • LOCATION: Waterbury, CT
  • SCHEDULE: Full-time (40 hours), 1st Shift, Monday, Wednesday, Thursday, Friday 8:00am - 4:30pm and Tuesday 10:00am - 6:30pm

The Role:

The ACT program is a best practice model of intensive community care with an emphasis on assisting clients to gain or improve skills for successful living in the community, an improving quality of life, and greater independence to pursue their goals through values of recovery.

 

Job duties include, but are not limited to:

 

  • Provides clinical, case management rehabilitative and recovery-oriented services to an assigned caseload of individuals with serious mental health and/or co-occurring substance and medical disorders.
  • Responsible for the intake, assessment, and ongoing evaluations of clients.
  • Work with clients and team members in the development of Individualized Recovery plans and supporting documentation.
  • Facilitates psychoeducation, therapy or skill building groups as well as provides limited individual and family therapy.
  • Provides culturally sensitive and evidence-based treatments such as DBT, EMDR, IDDT and Trauma treatment and works in the community for a majority of services. 
  • Makes community visits to assess individuals in psychiatric crisis situations and complete Emergency Certificates as needed. 
  • Independently represents the program at State and local hospitals, probate courts, and community settings.
  • Assists other team members in the development of recovery plans, may provide clinical consultation to team cases; may provide supervision to other team members and students.
  • Participates in and coordinates all treaters’ meetings, case conferences, critical incident reviews and other meetings as indicated.
  • Ability to work as a member of a multidisciplinary team, performs risk assessments, functional assessments, and possess computer technology skills. 
  • Demonstrates the ability to make appropriate and timely referrals and build relationships with service providers and natural supports. 
  • Documents all services in the medical record in a timely manner.
  • Maintains necessary productivity standards.
  • Demonstrates fidelity to ACT model in all aspects of work.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Licensure as a professional counselor in the State of Connecticut.

 

SPECIAL REQUIREMENTS

  • Incumbents in this class must possess and retain a license to practice professional counselor work pursuant to Sec. 20-195 (aa-jj) of the Connecticut General Statutes.
