We are seeking a Full-Time (30 hours/week) Youth and Family Case Manager to join our Safe Place program in Hillsboro, Oregon. What You'll Do
As a Youth and Family Case Manager, you will provide intake, placement, service planning, and case coordination for runaway and homeless youth receiving shelter services through Safe Place. Responsibilities include:
- Supporting youth and families through crisis intervention, mediation, skill‑building, and family support planning
- Assessing individual youth needs and coordinating referrals for education, employment, mental health, and other supportive services
- Arranging or providing transportation to school, medical appointments, assessments, court hearings, and other essential activities
- Maintaining ongoing contact with youth and families to support stabilization and connection
- Collaborating with the Safe Place team to ensure a safe, healthy, and youth‑centered environment
- Ensuring timely and accurate documentation of all services
- Maintaining client records and tracking systems to support program outcomes, progress monitoring, and compliance requirements
What We're Looking For
- Bachelor's degree in Social Work, Education, Psychology, Sociology, or a related field AND a minimum of two (2) years of relevant professional and/or lived experience
- Additional professional or lived experience may be considered in lieu of a Bachelor's degree
- Knowledge of group facilitation practices
- Strong boundaries, patience, and crisis response skills
Bilingual proficiency in Spanish and English is preferred, though not a requirement for the role
- Valid driver's license with an acceptable driving record
