Case Manager

Case Managers helps individuals with disabilities get the support they need to live fulfilling lives. They work closely with each person to create personalized plans, connect them to Medicaid services, and ensure they have the tools to make informed decisions about their lives.

Case Managers regularly checks in, meeting at least twice a year, to make sure the services are still meeting the person’s needs. They also handle important paperwork to track services, progress, and any changes in needs, keeping everything accurate and up to date. The role involves being an advocate, a planner, and a connector, always focused on helping individuals achieve their goals.
 

Responsibilities:

  • Develop support plans in collaboration with individuals, parents, or their legal guardians.
  • Find providers who are able to provide the services that matches the individuals based on their support plan.
  • Authorize services and develop budgets for authorized services.
  • Monitor the quality of services provided by checking in with the individual or their legal guardians.
  • Meet the individual in their own home or a safe location at a minimum once every 6 months. Though some situations require more meetings.
  • Provide updates to the individual, their legal guardians and providers whenever the requested services are rejected due to local, state or federal policy.
  • Adjust support plans and services as needed due to changes in needs and medical condition.
  • Advocate for the individual's needs, wants, and independence, including the right to take risks.
  • Complete documents relevant to social services.
  • Document all relevant work activities daily.
  • Conduct audits on your individual's case files to ensure compliance with local, state and federal policies.
  • Stay up to date to any service changes proposed by local, state or federal agencies.
  • Attend monthly team meetings.
  • Complete other duties as assigned.

Education and Experience:

  • Accredited four-year degree in social work, special education, psychology, nursing, human services, or related fields.
  • Or an accredited four-year degree with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Skills:

  • Effective written and verbal communication skills in English.
  • Ability to have difficult conversations regarding services to clients, guardians or service providers.
  • Ability to work with individuals with challenging behaviors.
  • Strong organizational and time management skills.
  • Capacity to work independently, while receiving support from supervisors and other supervising staff.
  • Quick learner, adaptable to a constantly changing environment.
  • Ability to handle challenges with a caring and compassionate approach.

Preferred Skills:

  • Bilingual proficiency.
  • Familiarity with Medicaid programs and services.
  • Additional one year of experience in education or treatment of people with developmental disabilities or related conditions, or completion of a relevant course.

Other Requirements:

  • Successful clearance of DHS background check.
  • Reliable vehicle and comfortable driving in Minnesota weather.
  • Possession of a Driver's License and car insurance.

Schedule:

  • Monday through Friday, 8:30 am to 5 pm.
  • Mandatory office work during onboarding.
  • 4x10 shift available after meeting performance criteria.

Sign On Bonus:

  • $1000 paid over a period of 6 months.

Job Type: Full-time

Salary: From $23.00 - $26 per hour based on relevant experience.

Expected hours: 40 per week
