Licensed Practical Nurse
We are seeking dedicated Licensed Practical Nurses (LPNs) to join our team . Duties
- Provide direct nursing care to residents, including medication administration, wound care, and vital signs monitoring, ensuring adherence to established protocols.
- Assist with activities of daily living such as bathing, dressing, feeding, and mobility support to promote independence and dignity among residents.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs, including dementia and Alzheimer's care.
- Monitor residents for signs of acute pain or distress, promptly reporting changes and assisting with pain management strategies.
- Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Support infection control procedures and sterile techniques during procedures like catheterization or specimen collection.
- Participate in discharge planning and family education to ensure continuity of care across settings such as assisted living or home & community care environments.
- Engage in ongoing training related to emergency medicine, ICU protocols, hospice care, or trauma center practices to enhance clinical skills.
Experience
- Valid LPN license with current state certification required; prior experience in long-term care or nursing home environments strongly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation and patient tracking.
- Experience working with individuals with developmental disabilities, dementia, or memory impairments including Alzheimer’s and other neurodegenerative conditions.
- Familiarity with medical terminology related to geriatrics, pain management, infectious disease care, and post-acute care practices.
- Medication administration including injections and venipuncture, and basic life support certification are essential.
