GENERAL PURPOSE
The Quality Assurance – Performance Improvement Coordinator position is responsible for the management of the Quality Assurance – Performance Improvement (QAPI) program and processes.
SUPERVISION RECEIVED
This position reports directly to the Nursing Home Administrator.
SUPERVISION EXERCISED
This position is not responsible for the supervision of other employees however may assign and review work of other staff as it relates to the QAPI program and activities.
ESSENTIAL DUTIES OF THE POSITION
Works with the Interdisciplinary Team (IT) and leadership to identify potential or current problems associated with the provision of care and services in the facility.
Develops quality assurance monitoring systems and departmental indicators to monitor performance of all departments to fulfill the QAPI plan.
Coordinates with Staff Development to train staff on the quality assurance monitoring systems. Performs periodic review of the systems to evaluate effectiveness.
Develops quality improvement plans. Leads quality improvement teams / Performance Improvement Projects (PIPs) and addresses goals identified by the quality assurance process.
Maintains compliance with QAPI in accordance with Section 6102 of the Affordable Care Act and Federal and State nursing home regulations.
Completes in-depth analysis of quality assurance data, including trending and root cause analysis.
Leads and completes quarterly Facility Assessment and uses results to make decisions regarding the Quality Assurance Plan and QAPI activities.
Supports the Risk Manager / Investigative Nurse in completing investigations and reports, including Incident Reports, to both regulatory bodies and facility insurance carrier. Leads Risk Meeting in the absence of the Risk Manager / Investigative Nurse.

REQUIRED MINIMUM QUALIFICATIONS
EDUCATION & EXPERIENCE - Possession of a high school diploma or equivalent; AND
At least three (3) years of full-time, professional work experience in a healthcare facility with experience in quality assurance and/or quality improvement measures. Experience in a long-term facility preferred.
CERTIFICATION/LICENSE - Possession of licensure as a Registered Nurse, issued by the Commonwealth of Pennsylvania preferred.
KNOWLEDGE, SKILLS, AND ABILITIES
Thorough knowledge of professional nursing principles, practices, and standards.
Knowledge of QAPI processes, data analysis, root cause analysis, goal setting, and project management.
Ability to establish and maintain effective working relationships with other team members.
Possession of excellent communication and interpersonal skills, both verbally and in writing.
Ability to prioritize tasks and do work under variable conditions and guidelines.
Possession of leadership/motivational skills with the ability to lead teams in completing PIPs and other QAPI activities.
Ability to operate a personal computer and secure information from established word and data processing, spreadsheet, database, graphics, and publishing programs.
Ability to keep records of activities and prepare reports from data.

Telephone, personal computer (including word processing and spreadsheet software), calculator, writing implements, fax machine, copy machine, and paper shredder.
PHYSICAL DEMANDS
While performing the duties of this job, the employee is frequently required to use hands to finger, handle, feel or operate objects, tools or controls; and reach with hands and arms. The employee is occasionally required to climb or balance; stoop, kneel, crouch, or crawl, walk sit and talk or hear.
The employee must frequently lift and/or move up to twenty-five (25) pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
