Reports to: Clinical Services Manager
Location: Denver
Salary Rate:
Level 1 (Entry level up to 2 years of direct experience): $24.04 per hour
Level II (3+ years of direct experience): $25.19 per hour
Bilingual premium for qualifying languages is 3.5% higher than starting salary if pass bilingual exam on first week of hire.

Position Summary:
Under the supervision of the Clinical Services Officer, this position provides an assessment of eligibility, insurance assistance and health education to clients seeking services through the Colorado Health Network Medical Clinic. Patient Navigation services will support CHN to better serve those disproportionately at risk for or living with HIV/HCV by improving their access to medical care. Bilingual preferred.
Essential Job Functions:
- Provide screenings, outreach and initiatives to promote PrEP, HIV/HCV program
- Coordinate and maintain an active caseload of PrEP, HIV/HCV patients
- Provide support and guidance to individuals seeking PEP by supporting their transition into PrEP
- Perform assessments, plan creation, and assist patients with access to medication and adherence to treatment as well as risk reduction plans
- Responsible for tracking, monitoring and occasional reporting of patient activities, including access adherence
- Complete necessary paperwork and data entry in a timely fashion including entering case notes
- Provide appropriate patient referrals to both internal and external agencies
- Collect necessary documentation required for program compliance
- Assist patients as they move through the service delivery system
- Assist with the elimination of barriers including, but not limited to: insurance, financial housing and transportation
- Provide emotional support for patients and their families, as needed
- Act as a liaison between patient and all relevant professionals, programs, and resources
- Educate patients and others about HIV disease process and prevention
- Assist with, and coordinate, community education and outreach initiatives as requested
- Maintain and upkeep of electronic health records (EHR)
- Participate and attend staff, department, team and community meetings upon request
- Perform effectively within the organization and interpersonally with co-workers
- Develop positive working relationships with other social service, medical and behavioral health providers
- Participate in fund-raising and advocacy events
- Must be available to work evenings and weekends
- Must be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities.
- Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues. Cultural Competency/Sensitivity.

Qualifications:
1-3 years’ experience working in healthcare setting. Experience using CAREWare, ARIES and EMR preferred. Experience working in a clinical setting
Education:
Bachelors or Masters Degree in: nursing, social work, sociology, psychology, counseling, or public health; or an equivalent combination of relevant experience and education required.
Software Utilized:
Experience working with basic office and database computer programs, including familiarity with Microsoft Outlook, Word, Excel, Access, SharePoint and the internet.
Knowledge, Skill and Ability:
Basic knowledge of HIV, STIs, PrEP and other prevention methods. Proactive problem solver who works well in multi-disciplinary setting. Ability to communicate well with medical providers and support staff. Strong verbal and written communication skills. Strong socio-cultural identification or work experience with MSM and LGBTQ+ preferred. Comfort discussing sexual health and risk reduction strategies. Proven organizational skills with high attention to detail. Ability to work well with diverse populations. Ability to work well within the organizational structure. Ability to speak Spanish desired.
Excellent communication, client services, and data collection skills. The appropriate person will be highly organized and detail-oriented. Must demonstrate accuracy, efficiency and superior organizational skills. Material and Equipment Directly Used:
Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine. Works with word processing software, spreadsheet applications, and presentation software. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax.
Physical Demands and Work Conditions:
Works in an office environment. Requires ability to lift up to 50 pounds of materials. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding and using a telephone and other office equipment. Works in an office environment. May travel via automobile or airplane to offsite locations or for CHN business. Must have and maintain a valid Colorado driver’s license and auto insurance on personal vehicle. Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding. The ability to sit, stand, and engage in extended periods of screen time or communication by phone is necessary to perform the essential duties of the role.
