We are seeking a detail-oriented and compassionate (Minimum Data Set) MDS Coordinator to join our healthcare team in our 80-bed, five-star facility. This vital role is responsible for ensuring accurate and timely MDS assessments, care planning, and regulatory compliance, while supporting the interdisciplinary team. This is a full-time, day shift position. Occasional scheduling flexibility may be required to meet assessment deadlines, attend meetings, or support clinical needs.


 

We welcome licensed nursing professionals to apply, with a strong preference and priority given to Registered Nurses (RNs).  Licensed Practical Nurses (LPNs) are welcome to apply; however, this position requires additional RN oversight for documentation and clinical review in accordance with regulatory requirements. 

Requirements

 

Responsibilities:

  • Lead and coordinate completion of all MDS assessments per RAI Manual guidelines.
  • Maintain and communicate MDS and Care Plan schedules to the interdisciplinary team (IDT).
  • Ensure accurate and timely MDS, CAA, and Care Plan documentation, including electronic submission to CMS/QIES.
  • Participate in Care Conferences, IDT meetings, and required committees.
  • Monitor resident census and clinical status, adjusting assessments and schedules as needed.
  • Oversee skilled services, including coverage decisions, cut dates, and scheduling to optimize reimbursement.
  • Communicate skilled census and documentation requirements to internal and external partners.
  • Support reimbursement optimization through accurate assessments and staff education.
  • Ensure regulatory compliance across the RAI process.
  • Provide backup support for Care Plan Nurse responsibilities and coordinate required nursing/IDT assessments.
  • Review documentation and resident care, following up with nursing staff as needed.
  • Communicate changes in skilled status or hospice enrollment to relevant departments.
  • Complete ICD-10 coding and collaborate with physicians, therapy, and medical records for accurate diagnosis coding.
  • Stay current with federal and state MDS regulations.
  • Assist with survey preparation, including census and roster documentation.

Qualifications:

  • Active and valid Nursing license in the State of Ohio (RN highly preferred)
  • Minimum of 1–2 years of experience in a healthcare setting, preferably in geriatrics or long-term care.
  • Experience with MDS/RAI process required.
  • Knowledge of Medicare and Medicaid regulations.
  • Experience with Electronic Health Records; familiarity with PointClickCare preferred.
  • Strong organizational, analytical, and communication skills.
  • Ability to work independently, prioritize tasks, and meet deadlines.
