Southside Community Health Services (SCHS) is seeking a Family Practice Physician to provide high-quality, patient-centered care for individuals and families in the South Minneapolis community. This role is part of a collaborative, mission-driven team dedicated to improving health through exceptional care, removing barriers to services, and promoting healthy lifestyles.
About the Role
The Family Practice Physician provides comprehensive primary care to patients across the lifespan, including preventive care, diagnosis and treatment of acute conditions, and management of chronic illness. This position works closely with an interdisciplinary care team to deliver integrated, culturally responsive care and support positive health outcomes for patients and families.
Key Responsibilities
- Provide comprehensive patient- and family-centered care including preventive services, health maintenance, acute care, chronic disease management, and patient education
- Collaborate with interdisciplinary care teams to coordinate patient care and support overall health goals
- Complete patient contact hours based on FTE and participate in rotating on-call coverage
- Maintain timely clinical documentation and manage EPIC in-basket items according to clinic policy
- Complete required medical forms, documentation, and administrative tasks in a timely manner
- Maintain credentialing, licensure, and compliance with clinical protocols and organizational policies
- Participate in peer reviews, staff meetings, and quality improvement activities
- Precept and supervise students as appropriate
- Assist with additional duties as assigned to support clinic operations

Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
- Board certified or board eligible in Family Medicine with no disciplinary actions
- Current Minnesota medical license
- Current DEA registration and BLS certification
- Experience with electronic health records (EPIC preferred)
- Community clinic or community health center experience preferred
- Bilingual (English/Spanish or Somali) preferred
