Camden Case Management provides personalized management services for individuals facing mental health and/or addiction challenges. Our team collaborates with clients at various stages of their healing journey, creating flexible, individualized reintegration plans within a comprehensive support structure. Our multidisciplinary team focuses on helping clients move toward functional independence by establishing achievable goals and fostering consistent progress.
CCM operates on the principles of:

We prioritize regular communication as a cornerstone for success and work alongside external clinical teams to ensure a cohesive approach to client reintegration and skill-building.
Role Overview
The Coach role provides targeted, skill-based support for clients facing specific challenges such as ADHD, OCD, autism spectrum disorder, PMDD, addiction recovery, and executive functioning difficulties.
Coaches help clients build practical life skills while collaborating with Camden’s Case Managers, clinicians, and families to support each client’s care plan.
This is a part-time, per-diem role based on client needs.
Key Responsibilities
- Meet with clients weekly in person (home or community settings)
- Provide coaching around organization, executive functioning, emotional regulation, and daily life skills
- Support clients with activities such as scheduling, errands, appointments, and goal setting
- Communicate regularly with Camden Case Managers and the care team
- Document sessions and observations in the Camden system
- Provide occasional updates to families when requested
- Follow Camden safety, confidentiality, and professional boundary guidelines
Requirements
- 4+ years experience in a relevant mental health or coaching field (e.g., ADHD, autism, OCD, addiction recovery, PMDD)
- Strong interpersonal and communication skills
- Ability to adapt strategies to meet individual client needs
- Previous coaching, mentoring, or direct support experience preferred
- Valid driver’s license and willingness to transport clients if needed

Hours may fluctuate depending on client demand, so we encourage team members to maintain additional employment.
