Pay Rate: $17.00 – $22.00 per hour (Per Diem)

Reports To: Supervisor/Manager, DDD Program
Position Overview
Priority Group Services is seeking a detail-oriented and compassionate Support Coordinator to assist in coordinating services for adults with developmental and intellectual disabilities (DD/ID). In this role, you will provide administrative and operational support to the DDD program by assisting with documentation, scheduling, communication, and service coordination. The Support Coordinator helps ensure that clients receive timely and appropriate services while maintaining compliance with state and DDD program requirements.
Key Responsibilities
- Assist Support Coordinators with managing caseloads for adults with developmental and intellectual disabilities by maintaining organized records and scheduling appointments.
- Prepare and maintain documentation for Individualized Service Plans (ISPs), progress reports, and compliance reviews.
- Accurately enter and update client information in DDD-approved databases and maintain clear service records.
- Communicate with clients, families, and service providers to coordinate appointments and ensure services are delivered as scheduled.
- Support the coordination of services across healthcare, vocational, residential, and community-based programs.
- Track service implementation to ensure alignment with clients’ Individualized Service Plans.
- Monitor client progress and assist in gathering information for reporting and documentation.
- Follow up with clients and providers to confirm services are delivered according to plan and DDD guidelines.
- Assist in distributing program resources, training materials, and service information to clients and families.
- Provide administrative support during crisis situations by coordinating resources and contacting appropriate providers.
- Ensure all documentation meets DDD standards and is audit-ready.
Qualifications
- High School Diploma or GED required.
- Associate’s degree in Human Services, Social Work, Psychology, or a related field preferred.
- Experience in administrative support, customer service, or social services preferred.
- Experience working with individuals with developmental or intellectual disabilities is a plus.
- Strong organizational skills and attention to detail.
- Excellent written and verbal communication skills.
- Proficiency in Microsoft Office Suite and database systems.
- Ability to maintain confidentiality and comply with HIPAA and privacy regulations.
- Ability to manage multiple tasks and work collaboratively with team members and service providers.
