Community Liaison

Position Overview

New Vision Health & Wellness is seeking a dynamic and relationship-driven Community Liaison to expand our community presence and develop strategic partnerships. This individual will serve as the face of our organization within schools, healthcare systems, community agencies, and referral sources.

The Community Liaison will play a key role in building sustainable referral pipelines, increasing service utilization, and strengthening collaborative partnerships aligned with our mission of delivering accessible behavioral health services.

This is a part-time independent contractor role with bonus potential tied to measurable partnership and referral outcomes.

Primary Responsibilities

Partnership Development:

  • Identify and build relationships with schools, hospitals, primary care offices, community organizations, faith-based groups, and social service agencies.
  • Schedule and attend in-person or virtual meetings to present New Vision’s services.
  • Represent New Vision at community events, health fairs, and networking meetings.

Referrals:

  • Develop and maintain a referral tracking system.
  • Create consistent follow-up processes to convert interest into active referrals.
  • Track and report monthly outreach metrics and referral outcomes.

Brand & Visibility:

  • Maintain strong knowledge of all New Vision service lines (therapy, case management, school-based services, group programming, etc.).
  • Assist with distribution of marketing materials and program updates.
  • Identify new market opportunities within the region.

Reporting & Strategy:

  • Provide monthly outreach reports to leadership.
  • Make strategic recommendations for growth and service expansion.
  • Maintain CRM or tracking documentation of all contacts and outreach activities.
