Family Practice Physician

Position Summary

The Family Practice Doctor (Family Physician) in a Federally Qualified Health Center (FQHC) provides comprehensive, patient-centered primary care to individuals and families across the lifespan, regardless of ability to pay. This role emphasizes serving medically underserved and vulnerable populations while supporting the mission and regulatory requirements of the Health Resources and Services Administration (HRSA), which oversees the FQHC program.

Physician delivers high-quality, culturally competent care in a community-based setting and works collaboratively within an interdisciplinary care team.

Key Responsibilities

Clinical Care

  • Provide comprehensive primary care services to patients of all ages (pediatric through geriatric)
  • Diagnose and treat acute and chronic medical conditions
  • Manage chronic diseases such as diabetes, hypertension, asthma, and cardiovascular disease
  • Deliver preventive care, including screenings, immunizations, and health education
  • Perform in-office procedures (e.g., suturing, skin biopsies, joint injections)
  • Provide women’s health services (e.g., Pap smears, contraception counseling)
  • Order and interpret diagnostic tests and labs
  • Prescribe medications and monitor treatment plans

Population Health & Quality

  • Participate in quality improvement (QI) initiatives and performance metrics reporting (e.g., UDS measures)
  • Support population health management strategies for high-risk patients
  • Collaborate with care coordinators, behavioral health providers, and case managers
  • Promote preventive care and health education within the community

Team-Based Care

  • Work within a Patient-Centered Medical Home (PCMH) model
  • Coordinate care with specialists, hospitals, and community resources
  • Supervise or collaborate with nurse practitioners, physician assistants, and medical residents (if applicable)

Compliance & Documentation

  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Adhere to FQHC regulations and HRSA compliance requirements
  • Support sliding fee scale policies and access-to-care initiatives

Work Environment

  • Community-based outpatient clinic setting
  • Collaborative, mission-focused team environment
  • Typically qualifies as a site for federal loan repayment programs (e.g., NHSC)
  • Typically full-time; on-call duties
  • Moderate physical activity, including standing for extended periods

Required Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Completion of an accredited Family Medicine residency program
  • Board-certified or board-eligible through the American Board of Family Medicine
  • Active, unrestricted state medical license
  • DEA registration
  • Current BLS and ACLS certification
