What We’re Looking For

 

Trillium Health Resources has a career opening for a Pediatric Complex Care Manager to join our team! 

The Pediatric Complex Care Management Supervisor oversees a skilled, integrated team of professionals who provide localized support within a managed care environment for members living with intellectual/developmental disabilities or traumatic brain injury, mental health, and/or complex medical needs. The Supervisor provides oversight to Care Managers supporting children birth to under age 21 with exceptional support needs to ensure members are linked to appropriate services and resources within their communities.

 

On a typical day, you might:

 

 

  • Supervise staff within the Pediatric Complex Care Team. 
  • Provide ongoing advocacy and education regarding Children with Complex Needs 
  • Ensure Care Coordination engagement, collaboration with members, and families to address issues, concerns, barriers to assessment, and services/supports. 
  • Ensure staff are utilizing person centered planning methods/strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual. 
  • Review referrals for TCM eligibility and make assignments as appropriate 
  • Provide support to Pediatric Complex Care Managers to resolve member issues/needs in a timely manner.

Qualifications

 

Required:

  • Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to complex members with I/DD or TBI, mental health and/or complex medical needs. OR 
  • A master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area or licensure as an RN, and three (3) years of experience providing care management, case management, or care coordination to complex members with I/DD or TBI, mental health and/or complex medical needs. OR
  • Equivalent combination of education/experience.
  • Must have a valid driver’s license
  • Must reside within Trillium’s Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.

 

 

Preferred:

  • Supervision experience. 
  • Experience working with children with complex medical and/or behavioral health needs, intellectual and/or developmental disabilities, providing care management, care/service coordination.
