Case Manager

Responsibilities

Provides case management and direct support to consumers both on site and in the community

Implements treatment plans, manages cases, and provides transportation and linkage.

Ensures the appropriate completion of all related documentation

Functions as an essential member of the team, assisting in shift coverage, participating program activities, and communicating with team members to cover all applicable duties

 

Qualifications

BSW in social work, BA/BS in psychology, or a related field

Two years of experience in mental health and human services

Valid Driver’s License
