Account Manager – Home Health & Hospice

Location: San Francisco, San Mateo, Marin, Contra Costa, Alameda & Santa Clara Counties
(We are hiring one Account Manager per county)

Position Overview

We are seeking a motivated and relationship-driven Account Manager to join our growing team. This is a territory-based sales role responsible for developing and maintaining referral relationships, increasing market awareness, and driving revenue growth within the assigned county.

The ideal candidate is passionate about healthcare, thrives in a relationship-building environment, and is motivated by performance-based compensation.

Key Responsibilities

Build and maintain strong relationships with physicians, hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and other referral sources

Actively develop new referral partnerships within assigned territory

Achieve and exceed assigned territory revenue and referral goals

Represent Vital Home Health and Hospice at industry events, networking meetings, and community outreach activities

Gather market intelligence and provide strategic feedback to leadership

Collaborate with clinical and operations teams to ensure smooth patient transitions and high satisfaction

Qualifications

1–2 years of sales experience in home health, hospice, or healthcare preferred but not required

Strong communication, networking, and relationship-building skills

Self-motivated with the ability to work independently in a territory-based role

Passion for healthcare and improving patient outcomes

Training will be provided for motivated candidates.
